HOW DO I ACCESS THIS SERVICE? Referrals to this programme can only be made via Social Services.

Each Social Work Office has been provided with a referral form, however they can also be accessed via our office in Bishop Street.

Referral forms should be completed by both the referral agent and the family and returned to the project manager via post or email.

Passport provides support in a group setting to children, aged 8-12 years, who are at medium to high risk of reception into residential care or custody, or who are assessed as being ‘a child in need.’
