Introduction
Extern provides a range of services for adults within the criminal justice system and individuals who are homeless who present with a range of complex needs.
The continuum of services provide intensive supervision monitoring and support to those individuals.
A whole systems approach and a range of evidence based models are utilised to ensure the safety of the individual and others, with particular emphasis on multi-disciplinary, multi-agency working arrangements which prioritises risk assessment, risk management and community safety.
This approach includes the provision of 24 hour monitoring with high support programmes within accommodation services, Community support in resettlement, support and managing individuals within their community and training and work experience within a secure work skills and Social
Economy environment.
Person centred planning forms the basis for work undertaken with service users, placing them at the centre of the change management process.
